As a participant in the activities of the City and County of San Francisco Recreation & Park Department, I hereby waive and release any claims I may have, now or in the future, against the City and its officers, employees, contractors, servants and agents (hereafter referred to as The City) arising from injuries to me, or damage to my property, sustained while I am: (1) at The City's facilities, (2) participating in the City's activities, or (3) being transported to or from the City's facilities or activities, regardless of whether such injuries or property damage is caused in whole or in part by the City's active or passive negligence.

In the event of an injury to my child, I hereby give the City permission to arrange transportation for my child to a medical facility, and/or provide my child with emergency treatment or first aid.  I understand that the City does not assume any responsibility to take any of these actions.

This waiver and release shall be valid for the duration of the sessions in which my child is enrolled.  I have carefully read this waiver and release and agree to the terms stated.  By registering with US Futsal, SF Rec and Park and by signing roster form I agree to terms stated.

Procedure for Registration Online and also With Jose Coronado League

Step 1:
If you are a returning coach, please click on "Sign In" on the top right corner of the website.  Enter your email address and password and click on "Remember Me" so you don't have to sign in again.  If you have forgotten your password, enter your email address and click on "I Forgot My Password! Please Email Me A Reminder" and your password will be emailed to your email address.

If you are a new coach, please click on "Create Your Sign In" on the top right corner of the websites.  Enter your name, email address, choose a password, etc. and click on "Create Your Sign In".

All information entered last year (including players and coaches pics) is available for all coaches to transfer to this years' roster.

THIS YEAR, COACHES OR ASSISTANT COACHES DO NOT PAY ANY REGISTRATION FEE !!


Step 2:
You can find the Registration Center in the Red Menu Bar above.  You will be registering under Coach/Team Registration.  Please click on
 
Coaches - Register Your Team Here!



  • Click on "Add A New Team"
  • Enter your team's name and select your team's division for this year.
  • Click the "Next Step" button.
  • Review/Complete your information
  • Click the "Next Step" button.
  • Enter the information for an additional coach, if any.
  • Click the "Next Step" button or the "Skip This Step - I am the only Coach" button, as appropriate.
  • If you registered a team last year, you may select it from the drop down list and click on the "Copy the Selected Prior Team's Roster To The Team Roster Below" button to copy your roster from last year's team to this year's team roster.  You can edit, add or delete as your choose.
  • Once you are finished with your roster, please click on the "Save My Team Roster/Next Step" button.
  • You can review all of the information you have entered for your team and edit it if necessary. 
  • Click on "I Agree" at the bottom of the window and click on the "Submit" button.

 

This year, coaches can do all the registration themselves, including entering all pertinent info for teams and players, uploading pics, and printing waiver forms.  Each coach will then have to print out completed roster form, and turn it in to league official along with fees and other pertinent documentation.  You may print out all waivers forms (coaches and players) and mail them to the address on the waiver.  If you include your new players' email addresses, email invitations will be sent to them inviting them to complete their portion of the online registration process.

The league is limited to nine (9) teams per division.  Extra teams will be placed on a waiting list.  First come first served.  Gym space is limited, so register soon!

Each team is limited to a maximum of twelve (12) players on their roster with an absolute minimum of eight (8) players.  You can also have a maximun number of 3 coaches registered.

Additions and deletions to the roster must be done in writing with the league coordinator and must be on file before a player is eligible to participate.  Click here to download a Futsal Add/Drop Form (PDF).

ALL PLAYERS, COACHES AND ASSISTANT COACHES (YOUTH AND ADULTS TEAMS) WILL BE ISSUED FUTSAL MEMBER PLAYER AND COACHES PASSES.

It is mandatory for all new and or returning coaches to register their teams with the United Sports League (USL) and with United States Federation of Futsal (FEFUSA).  Please click here to go to the Registration Center now.

NO TEAM WILL BE ALLOWED TO PARTICIPATE UNTIL REGISTERED ONLINE!

Season

  • Youth League will run on Sundays and will start on January 11, 2009.
  • Adult League will run on Monday nights, and will start on January 12, 2009.
  • The League will run through end of March (10 weeks).
  • Game Sites Include:  Upper Noe Rec Center, Mission Rec Center, St Mary's Rec Center, Glen Park Rec Center, Eureka Valley Rec Center, Sunset Rec Center, Richmond Rec Center, Palega Rec Center and Ocean View Rec Center.
There will be no games scheduled for:
  • Sunday Jan 18, 2009 or Monday Jan 19, 2009 due to Martin Luther King Day.
  • Sunday February 15 and Monday February 16, 2009 due to President's Day Holiday.
Day light savings starts on Sunday March 8, 2009.
Clocks are set ahead one hour!

ALL GAMES WILL BE PLAYED AS SCHEDULED.
THERE WILL BE NO CHANGES.

Youth Divisions

No youth is allowed to play with youth and adult teams at the same time.

  • U7, U8, U9, U10, U11, U12, U13, U14, U15, U16, U17, U18 Boys Rec Divisions
  • U7, U8, U9, U10, U11, U12, U13, U14, U15, U16, U17, U18 Boys Premier Divisions.

  • U7, U8, U9, U10, U11, U12, U13, U14, U15, U16, U17, U18 Girls Rec Divisions.
  • U7, U8, U9, U10, U11, U12, U13, U14, U15, U16, U17, U18 Girls Premier Divisions.
All divisions will have post season games with the exception of the U7 and U8 divisions.  They will receive participation awards.

Age cut-off date is August 1, 2009.
